REGISTRATION PROCESS

Our pre-registration software, EPION, assists us in giving you the best experience possible.

How it works:

Please be on the look out for a link via text message and email 3 days prior to your appointment. This message will contain a link asking you to verify your information. You will be asked to fill out your demographic information, upload a picture of your insurance card and driver’s license, sign consent forms and fill out a medical history questionnaire. If you have not filled out your forms 1 day prior to your appointment, you will then receive an additional text reminding you to complete your forms.

Once you have completed all the forms, you will be all set!

Where does the message come from?

The text and email messages are sent from our trusted partner, Epion Health, and enable us to deliver a private, secure and personalized check-in experience. The text message will be coming from 855-771-0817.

Yes. However, once you have completed the initial registration, the next time will be much quicker because Epion Health saves the answers.


No. There is no app – you simply click the link provided in the text message or email. This allows you to securely answer any registration and medical questions necessary for your appointment.
